Summer Registration

 

The following steps must be completed in order to re-activate your existing Student ID number.

Step 1 - Gather Information

Start by gathering together any information that will be needed during the registration process.

  • You will need to provide an up-to-date immunization record for the 2024-2025 school year. 
  • You will also need to update the Student's Health Inventory records with detailed information regarding Any medications, medical conditions, and allergies that the School Nurses will need to be aware of.
  • List of emergency contacts for the student including Email and Phone numbers.

Please Note: Birth Certificate and Social Security Card will not be needed to reactivate an existing Student ID number. Proof of Residency will only be needed if the student will be attending SPS for the regular 2024-2025 school year.

Step 2 - Complete the Health Inventory Form

Explore! Staff will not be able to re-activate your Student ID number until they have an up-to-date Student Health Inventory, and Immunization Record on file. Click on the button below and complete the Health Inventory form. This will be required for Step 5.

Step 3 - Immunization Records

Click the buttons below to download the Immunization Requirements for the 2024-2025 School Year. Students will not be able to attend Explore classes unless they have received the appropriate immunizations.

Step 4 - Update SPS Enrollment Application

You will need to update the Student's enrollment information. This may be accomplished by completing a new Enrollment Online Application. Additionally, you may upload the required Immunization Records and Health Inventory as attachments to the Enrollment Application.

Please note: The Enrollment Online Application is different from the website used to register for Explore Summer courses. A new enrollment application must be completed for inactive students even if the student has previously attended SPS in the past.

Step 5 - Contact Staff

Please submit a completed Health Inventory form and Immunization record by one of the following methods:

  • adding as attachments to the Enrollment Application in step 4
  • faxing the forms to 417-523-1695
  • emailing scanned copies of the documents to immunizations@spsmail.org
  • bring in person to the school

 

Additionally, if the student has special dietary needs or special health concerns, i.e. severe asthma, diabetes, seizures, etc., please contact Health Services at 417-523-1630.

 

Please Note: Please allow 1-2 business days for SPS staff to review the information and reactivate the Student ID if the information has been faxed or emailed.

Step 6 - Register for Explore!

Once your Student ID number has been reactivated, you should be ready to log in to the Explore registration site and register for classes.
